胸锁乳突肌瓣在腮腺良性病变手术中的应用

APPLICAFION OF THE STERNOCLEIDOMASTOID MUSCLE FLAPS IN PAROTIDECTOMY

目的观察腮腺良性病变手术中Ⅰ期应用胸锁乳突肌瓣旋转修复的可行性及临床效果。方法将我科2005年6月--2008年10月收治的35例腮腺良性病变手术病例作为改良手术组，对照组为本院2000年--2005年接受常规腮腺良性肿瘤手术并获得随访的患者36例。改良手术组接受腮腺切除术，同期胸锁乳突肌瓣旋转修复术中缺损，对照组只接受腮腺切除术。结果住院期间发生涎漏者改良手术组0例，对照组9例，随访3个月～4年，改良手术组出现术区凹陷畸形0例，Frey’s综合征1例，对照组出现术区凹陷畸形34例，Frey’s综合征11例。结论应用胸锁乳突肌瓣旋转工期修复腮腺良性病变术中的缺损，能明显改善术后凹陷畸形，同时有效减少涎瘘和Frey’s综合征的发生率。

Objective To obeserve the feasibility and clinical effect of appplying the sternocleidomastoid muscle flaps in parotidectomy of parotid with benign lesion. Methods The modified group consisted of 35 patients who underwent both parotidectomy of parotid with benign lesion and restitution with sternocleidomastoid muscle flaps from Jun 2005 to Dec 2008, while the control group consisted of 35 patients who underwent parotidectomy of parotid with benign lesion from 2000 to 2005. Results No .salivary fistula occurred in the modified group while 9 eases in the eontrol group. The 3 months to 4 years of follow- up surveys showed that Frey＇s Syndrome occurred in one case in the modified group , whereas sunken cheeks occurred in 34 cases and Frey＇ s Syndrome occurred in 11 eases in the control group. Conclusion Appplying sternocleidomastoid muscle flap in parotideetomy of parotid with benign lesion appears to be an effective method for the prevention of salivary depression, fistula and Frey＇s Syndrome.
