内镜下rAd—p53注射联合动脉灌注治疗进展期胃癌

Clinical Observation of endoscopy-guided injection of rAd-p53 plus regional artery infusion chemotherapy on advanced gastric carcinoma

目的观察内镜下瘤体注射重组人p53腺病毒(rAd—p53)联合区域动脉内灌注化疗药物治疗进展期胃癌的临床疗效。方法经胃镜及病理确诊的进展期胃癌、失去手术或不愿手术的胃癌患者5例应用内镜下rAd—p53瘤体注射,剂量3x1012VP,术后3天行肿瘤区域动脉药物灌注,顺铂(DDP,DDC40mg/m2),每月1次治疗,3次为一个疗程。结果治疗组显效2例,有效3例,总有效率100.0%。结论内镜下rAd—p53瘤体注射联合胃窦肿瘤区域动脉灌注化疗对失去或不愿手术的进展期胃癌患者而言,是一种疗效满意、副作用少方法。

Objective To evaluate the efficacy and safety of the endoscopy-guided intratumoral injection of recombinant adenovirus-p53 （rAdp53） in combination with regional artery infusion chemotherapy for treatment of gastric carcinoma. Methods From January 2008 to January 2009,intratumoral rAdp53—injection guided endoscopy in combination with regional artery infusion chemotherapy was used for treatment of 5 gastric carcinoma cases. rAd-p53： 3 x1012 VP（viral particles）. once a month,DDP,40mg/m2,3 times each course of treatment The change in tumor volume and adverse effects were observed,and curative effect was evaluated by Endoscopy. Results Among the 5 patients,CR was achieved in two case,PR was achieved in there cases,limiting toxicity nor adverse event was not observed,except transient fever after rAd-p53 injection. Conclusion Treatment with intratumoral injection of Adp53 plus regional artery infusion chemotherapy on advanced gastric carcinoma is safe and effective.
