全身热疗治疗原发性肝癌和转移性肝癌

The effect of whole body hyperthermia (WBH) on primary hepatic cancer or hepatic metastases

目的了解全身热疗对原发性肝癌和转移性肝癌的影响。方法 5例患者,1例为原发性肝癌,其余4例为转移性肝癌。采用体外循环的的方法,即从股动脉插管引出血液,泵入热交器循环加热后从股静脉输入,中心体温升到41.8～42.0℃时降低热交换器内循环的血流温度缓慢升温至42.5℃时,给予维持。热疗时间根据患者的血压和心率的变化作相应调整。结果原发性肝癌患者3次热疗后短期内肝内肿块有缩小,2例热疗后肝内肿块无明显变化,1例热疗后肝内转移灶增大,1例热疗后急性左心功能衰竭死亡。结论全身热疗对原发性肝癌和转移性肝癌有效,全身热疗有一定的并发症。

Objective to study the effect of whole body hyperthermia （WBH） on primary hepatic cancer or hepatic metastases. Methods One patient with primary hepatic cancer and five patients with hepatic metastases were treated with WBH. WBH was delivered by using extracorporeal devices （Blood was drained fi＇om femoral artery and pumped through heat exchanger system, then reinfused into femoral vein）, the time and core temperatures of WBH were depended on the variation of blood pressures and heart rates. Results the size of the primary tumor reduced from 101.4 × 86.8 mm to 9.0 × 7.0 mm after three treatments with WBH, two patients hepatic metastases kept stable, the value of hepatic metastases in one patient increased, and one patient died from left heart failure. Conclusion Effects of WBH on primary hepatic cancer and hepatic metastases were observed. And side effects occurred after WBH.
