“间隙手术”在胃肠癌根治术中的应用（附26例报告）

The application of interspace concept in the radical resection of gastrointestinal cancer （a report of 26 cases）

目的探讨“间隙手术”在胃肠癌根治术中的应用。方法观察直肠癌Miles手术4例，Dixon手术5例，乙状结肠癌6例，右半结肠癌根治5例，胃窦部癌D。手术6例的手术时间，出血，术中游离结扎个数。结果Miles手术出血（97．50±12．58）mL，手术时间（135±12．91）min，结扎数（20±1．41）个；Dixon手术出血（51．67±13．29）mL。手术时间（115±8．37）min，结扎数（22．33±1．37）个；乙状结肠癌手术出血（48．33±9．83）mL，手术时间（103．33±12．11）min，结扎数（17．00±0．89）个；胃窦部癌D2手术出血（126．67±8．17）mL，手术时间（231．67±18．35）min，结扎数（27．83±6．24）个。结论“间隙手术”是一个重要的手术理念，尤其对于肿瘤外科医生。

Objective To study the application of interspace concept in the radical resection of gastrointestinal cancer. Methods 4 patients undergoing abdomino-pcrineal excision,5 patients undergoing low anterior resection,6 patients undergoing sigmoid colon cancer radical resection,5 patients undergoing right hemicolon cancer radical resection and 6 gastric cancer D2 lymph node dissection were performed to observe the operation time, bleeding and the number of ligation. Results The operation time, bleeding and the number of ligation were （135±12.91）min, （97.50±12.58）mL, （20±1.41） during abdomino-perineal excision; （llS±8.37）min, （51.67±13.29）mL, （22.33±1.37） during low anterior resection; （103.33±12.11）min, （48.33±9.83）mL, （17.00±0.89） during sigmoid colon cancer radical resection; （231.67±18.35）min, （126.67±8.17）mL, （27.83±6.24） during gastric cancer D2 lymph node dissection. Conclusions The interspace concept is an important principle in surgery, especially for oncosurgery.
