数值化便潜血试验在大肠疾病诊断中的应用

The evaluation of the quantitative immunochemical fecal occult blood test in the diagnosis of colorectal diseases

目的探讨数值化便潜血试验在不同大肠疾病中的临床诊断价值。方法对在我院内镜中心同时接受胃镜和结肠镜检查的患者在检查前留其大便标本后使用数值化法和免疫胶体金法进行检查。结果共检测标本237例,同时进行了两种试验方法的配对标本136例,数值化法在结直肠癌中的阳性率明显高于免疫胶体金法,分别为91.2%和55.9%,差异有统计学意义(P<0.01);在炎症性肠病分别为87.5%和62.5%,大肠息肉9.1%和4.5%,正常大肠2%和6%,阳性率均无显著差异(P>0.05)。结论与免疫胶体金法相比,数值化便潜血试验对结直肠癌有更高的诊断价值。

Objective To investigate the clinical value of the quantitative immunochemical fecal occult blood test（FOBT） in different colorectal diseases.Methods Patients who were going to undergo gastroscopy and colonoscopy simultaneously accepted the quantitative immunochemical FOBT and the colloidal gold immunochemical FOBT.Results The number of total sample was 136.The positive rates of the quantitative immunochemical FOBT and the colloidal gold immunochemical FOBT in colorectal cancer were 91.2% and 55.9%,inflammatory bowel disease were 87.5% and 62.5%,colorectal polyp were 9.1% and 4.5%,and normal group（normal appearance of colorectal mucosa） were 2% and 6%,respectively.The positive rate of the quantitative immunochemical FOBT in colorectal cancer was significantly higher than that of the colloidal gold immunochemical FOBT（P0.01）,while no statistical difference was found in the other diseases（P0.05）.Conclusion Compared with the colloidal gold immunochemical FOBT,the quantitative immunochemical FOBT has more sensibility in the diagnosis of colorectal cancer.
