帕金森病脑白质高信号的磁共振研究 被引量:3

摘要 目的:探讨帕金森病(PD)患者脑白质高信号(WMH)的磁共振影像特点及其与临床分级的关系。方法:对PD和正常对照组,采用核磁共振TW1、TW2和FLAIR成像技术观察影像学表现;对帕金森病患者的年龄、病程和分级与影像学的相关性进行分析。结果:PD患者可出现脑白质的异常高信号,出现率与对照组比差异无显著性;对照组和PD组WMH阳性患者的平均年龄高于WMH阴性患者,WMH阳性PD患者的病程与WMH阴性者比差异无显著性,而WMH阳性患者的分级较WMH阴性患者高;PD患者中深部脑白质高信号(DWMH)阳性者和脑室周围高信号(PVH)阳性者的年龄均较阴性者大,差异有显著性(P<0.01);DWMH阳性者的病程、分级与DWMH阴性者比差异均无显著性,PVH阳性者的病程与PVH阴性者比差异无显著性,但分级显著高于PVH阴性者。结论:PD患者的WMH与年龄、病程和临床分级有一定的相关性,WMH的病理机制有待进一步研究。 Objective: To explore the correlation between white matter hyperintensities (WMH) and the clinical features of Parkinson’s disease (PD) when examined with magnetic resonance imaging (MRI).Methods: The imaging appearances of MRI of PD and the normal controls were observed with the technique of TW1,TW2 and FLAIR.The correlation between imaging features and age,course of disease,clinical grades was analyzed.Results: The abnormal white matter hyperintensities of the brains for PD could be found.The frequency had no difference compared to the controls.For the control and PD,the average age of positive WMH was older than that of negative WMH.The course of disease of PD with WMH had no significant difference compared to PD without WMH.The clinical grades of PD with WMH was higher than those of PD without WMH.The patients with DWMH and PVH were older than those without DWMH and PVH (periventricular hyperintensities).The course and clinical grades of PD with DWMH had no significant difference compared to those without DWMH.The course of disease of PD with PVH had no significant difference ceompared to that of PD without PVH ;however,the grades of the patients with PVH were higher than those without PVH.Conclusion: There is definite correlation between the WMH and age,the course,the grades of PD.The pathogenesis of WMH need to further investigate.
