摘要
目的探讨煤工尘肺患者脉压水平对冠心病的影响。方法选自2007年1月—2008年12月某煤矿现已脱离粉尘作业的男性煤工尘肺患者260例为研究对象(尘肺组),以某建筑公司无粉尘作业史的男性工人260例为对照组。对尘肺组与对照组进行一般情况调查、疾病史调查,同时测量血压、身高、体重,检查心电图、心脏超声及血脂。资料整理后,用SPSS13.0统计软件包分析并处理数据,尘肺组与对照组比较采用t检验和χ2检验,以P<0.05为有统计学意义。结果尘肺组收缩压[平均(131.67±15.94)mmHg]、舒张压[平均(75.62±9.83)mmHg]水平均低于对照组[平均(137.01±18.69)mmHg、(84.25±10.31)mmHg)];脉压[平均(55.86±13.96)mmHg)]水平高于对照组[平均(52.83±15.02)mmHg];尘肺组冠心病患病率高于对照组,差异有统计学意义(P<0.05);不同期别煤工尘肺患者冠心病患病情况比较,差异无统计学意义(P>0.05);无论脉压<60mmHg或≥60mmHg,尘肺组冠心病患病率均高于对照组(P<0.05)。结论煤工尘肺患者冠心病患病率高于对照组,无论是尘肺组还是对照组,患冠心病的危险性均随脉压水平的增加而增加。未发现尘肺期别对尘肺患者冠心病的患病率有影响。
[Objective]To study the influence of pulse pressure level of pneumoconiosis coal workers on coronary heart disease.[Methods]Totally 260 male coal workers with pneumoconiosis who had been separated from dust work were selected as study objects (pneumoconiosis group),260 male workers without dust exposure from a construction enterprise were selected as control group.The general investigation and disease history investigation were performed on pneumoconiosis group and control group,as well as simultaneous survey of blood pressure,height,body weight,inspection electrocardiogram,cardiac ultrasound and the blood fats.After data compilation,SPSS13.0 was used to analyze and process the data,T-test and χ2 test were adopted in comparison of pneumoconiosis group and control group,taking P0.05 as statistical significance.[Results]Pneumoconiosis group’s systolic pressure [average (131.67 ± 15.94) mm Hg] and the diastolic pressure [average (75.62 ± 9.83) mm Hg] was lower than the control group equally (average 137.01±18.69 mm Hg,average 84.25±10.31 mm Hg );the pulse pressure [(55.86±13.96) mm Hg] of pneumoconiosis group was higher than that of the control group [average 52.83±15.02 mm Hg];the coronary disease prevalence of pneumoconiosis group was higher than that of the control group,the difference was significant (P0.05);comparing the situation of coronary heart disease of coal worker pneumoconiosis patient in the different periods,the difference was not significant (P0.05);regardless of pulse pressure 60 mm Hg or pulse pressure≥60 mm Hg,prevalence for coronary heart disease of pneumoconiosis group was higher than that of the control group (P0.05).[Conclusion]The coronary disease prevalence of pneumoconiosis group is higher than that of control group.No matter which group is,the risk of coronary heart disease increases along with the increasing of pulse pressure level.And the influence of the pneumoconiosis period on the prevalence of coronary heart disease of pneumoconiosis patient has not been discovered.
