胸腺瘤的CT诊断及病理对照分析

CT Diagnosis and Pathological Analysis of Thymoma

目的研究胸腺瘤的CT表现及病理表现,研究CT对胸腺瘤的分类、分期和指导临床治疗的价值。材料与方法回顾性分析41例胸腺瘤的临床资料与CT表现,并进行病理对照分析。结果胸腺瘤病理分为良性胸腺瘤、恶性胸腺瘤Ⅰ型、恶性胸腺瘤Ⅱ型。CT表现:纵隔肿块;纵隔胸膜侵犯;胸膜种植;肺侵犯;心血管侵犯;心膈角区及腹腔受累。本组病例CT对纵隔肿块、肺侵犯、心膈角区及腹腔受累显示较好,但CT对纵隔胸膜侵犯显示较差。肿瘤的大小对判断病变恶性程度有一定价值,肿瘤越大,恶性程度越高;而瘤内钙化对判断恶性程度可能无价值,肿瘤内出现钙化不排除侵袭性胸腺瘤的可能。结论CT能准确显示恶性胸腺瘤周围侵犯范围、胸膜种植及远隔转移,可进行分期和决定治疗方案,并对分类及预后做出评估。

Objective To study the CT manifestations of malignant thymoma for classification evaluating, staging and treatment planning. Materials and Methods We reviewed CT scans of 41 cases confirmed by surgery and pathology and researched the correlation between them. Results Thymomas were devided into benign, malignant type I and II. The CT findings were：mediastinal mass;mediastinal pleural invasion;pleural seeding ;lung invasion ;cardiovascular involvement ; invasion of pericardiophrennic corner and abdomen. Conclusion CT provides the most reliable assessment of the invasion and extra-mediastinal metastases, it allows staging and treatment planning, and has value in evaluating classification and prognosis.
