目的报道应用游离股前外侧皮瓣修复足踝部大面积皮肤软组织缺损的临床效果，进一步探讨游离股前外侧皮瓣的临床应用及足踝部大面积皮肤软组织缺损的修复。方法2006年6月至2009年4月对30例足踝部大面积皮肤软组织缺损患者应用游离股前外侧皮瓣、肌皮瓣进行修复，以旋股外侧动脉降支、横支及蒂重建皮瓣血供，以股外侧皮神经重建感觉。结果术后皮瓣全部成活，足踝创面得到有效修复，随访3个月到2年，7例臃肿3个月后整形，皮瓣外观及弹性良好，两点辨别觉12-14mm，足踝功能恢复良好。结论游离股前外侧（肌）皮瓣营养血管相对恒定，血供丰富，切取面积大，部位隐蔽，是修复足踝部大面积皮肤软组织缺损有效的手术方法。

Objective TO study the clinical effect of free anterolatrral femoral skin flap to repair big area shin defects of foot and angle. Methods Free anterolateral femoral skin flap was designed and harvested for free transfer to repair big area shin defects of foot and angle. Thirty cases of free anterolateral femoral skin flap were used Frorm June 2006 to April 2009. Cutaneous nerves was included in the flap and transferral To restore sensation. Results All the transferred flaps survied. Swelling of the flap Occurred in 7cases and venous crises were found in time in 3 cases. All cases were Followed up 3 to 24 months, the outline and funcation obtained satisfactory restore at last. Conclusion The artery of Anterolateral femoral skin flap has constant vascular anatomy And rich coetaneous perforators. It is ideal method to repair big area shin defects of foot and angle
