B型钠尿肽在急性冠脉综合征早期介入治疗中对无复流、慢血流现象的预测价值 被引量:4

摘要 目的介入治疗后发生无复流、慢血流现象直接影响急性冠脉综合征患者的临床疗效和预后,分析急性冠脉综合征发生无复流、慢血流患者早期B型钠尿肽(BNP)的水平,并以其作为无复流、慢血流的预测因素。方法入选急性冠脉综合征患者152例,均在发病12小时内完成介入治疗。所有患者于介入术前从周围静脉中留取血样测定BNP,依据术后TIMI血流分级分组,TIMI血流0~2级为无复流、慢血流组,TIMI血流3级为正常组,比较组间BNP有无差异,并随访近期主要不良心血管事件(MACE)。结果发生无复流3例,发生慢血流10例,无复流、慢血流组BNP水平明显升高。以BNP水平100pg/ml为临界值,术前BNP≥100pg/ml的患者术后发生无慢血流的比例明显高于低水平BNP组,特异性为84.9%,阴性预测值为95.9%。1个月后随访发现,无复流、慢血流组LVEF明显下降,3个月内MACE发生率无复流、慢血流组明显增加。结论在急性冠脉综合征患者中,发生无、慢血流患者术前BNP水平明显高于未发生者,以BNP水平为100pg/ml为临界值,对术后发生无慢血流有一定预测作用;无慢血流的发生影响患者近期预后。
