跟骨关节内骨折的手术治疗

Operative Treatment of Intra-articular Calcaneai Fracture

目的:探讨跟骨钢板内固定治疗跟骨关节内骨折的效果。方法:采用钢板内固定治疗跟骨关节内骨折31例,男21例,女10例;单侧骨折19例,双侧骨折12例共50足;年龄(21～65)岁,平均39岁;按Sander分型:其中:Ⅱ型4足,Ⅲ型25足,Ⅳ型21足。对临床结果进行分析总结。结果:31例术后均获随访,时间(6～32)个月,平均18个月。骨折全部愈合。采用Maryland足部评分标准评价手术效果:优28足,良15,可6足,差1足,优良率86%。术后并发症:切口延迟愈合3例,1例皮肤感染,1例皮肤坏死。结论:跟骨钢板内固定治疗跟骨关节内骨折是目前治疗跟骨关节内骨折较为理想的方法之一。

Objective：To explore effect of intra-articular fractures of calcaneus with calcaneal plate.Methods：Thirty-one cases of intra-articular fractures ofcalcaneus were classified by the standard of Sanders,including 4 cases of type Ⅱ.25 cases of type Ⅲ and 21 case of type iV.Among them,21 cases were male and 10cases female;the patients were 21 to 65 years old with average 39 years of age.All the patients were treated with internal fixation of calcaneal plates.Results：All eases were follow up for （6-32）months with average 18months,all eases recovered with good healing of fracture,according to Maryland criteria,an excellent outcome was found in 28 feet,good in 15,fair in 6,poor in 1,the excellent and good rate was 86%.There occurred 3 delaying healing, linfection and 1 skin of incision.Conclusion：Open reposition and internal fixation with calcaneal plate is an ideal method in treatmentof intra-articular fractures of calcaneus.
