三腰椎横突综合征260例的曲安奈德注射治疗观察

Clinical Observation of 260 Cases of Third Lumbar Vertebra Processus Transversus Syndrome by Injecting Triamcinolone

目的:探讨痛点局部注射曲安奈德治疗三腰椎横突综合征的临床效果。方法:260例三腰椎横突综合征患者,用8号穿刺针,每一痛点取曲安奈德5～10mg+2%利多卡因2～3m1+生理盐水2～3ml,分别注入三腰椎横突、骶棘肌起点、臀中肌起点、髂前下棘、耻骨结节等临床检查出之痛点处。间隔7～10日治疗1次。结果:经1～4次治疗,显效221例,疼痛完全消失,活动恢复正常。有效26例,疼痛明显缓解,可以忍受,不影响日常工作及生活。无效13例,疼痛无明显缓解,或暂时缓解数日后又复发。显效、有效、无效分别占:85%(221/260)、10%(26/260)、5%(13/260),总有效率:95%,无副损伤及其他明显不良反应。结论:痛点局部注射曲安奈德治疗三腰椎横突综合征方法简便,疗效显著,无副损伤,为治疗三腰椎横突综合征的理想方法。

Objective： To investigate the clinical effect of treating third lumbar vertebra processus transversus syndrome by injecting triamcinolone on pain spot partially. Methods：260 cases of third lumbar vertebra processus transversus syndrome were injected into on each pain spot with triameinolone 5-10mg combining with 2% lidocaine 2-3ml and normal saline 2-3ml by No.8 puncture needles at the beginning binding of third lumbar vertebra and multifidus or the beginning binding of gluteusmediusmuscle or anterior inferior spine or the beginning binding of tuberculumpubicum separately so on each time per 7-10days respectively, which were diagnosed to be pain spot by clinical examination.Results： 221 cases were effective significantly with ache vanishing completely and the activity restoring normally after I-4 treating courses. 26 cases were effective with ache alleviating significantly and becoming endured to it, not interfering with the routine work and life. 13 cases were invalided by ache alleviating not significantly or recurring to be ach after alleviates temporarily for several days. The percent of significant effective, effective, invalided effective cases were 85%（221/260）, 10%（26/260）, 5%（I3/260） separately. Total effective percentage was 95% without side damage and other obvious untoward effects. Conclusions：It is simple, ideal curative method to treat third lumbar vertebra processus transversus syndrome by injecting triamcinolone on pain spot partially without side damage.
