86例颞叶癫痫患者的临床特征和脑电图研究

A study of the clinical characters and EEG of 86 patients with temporal lobe epilepsy

目的探讨颞叶癫痫(temporal lobe epilepsy,TLE)的临床特征和脑电图(electroencephalogram,EEG)的特点。方法对86例已确诊为TLE的患者均作EEG和视频脑电图(V ideo-EEG,VEEG)检查,并结合临床症状进行分析。结果 86例患者的EEG正常39例,异常47例。VEEG正常5例,异常81例。其中左侧颞叶痫样放电30例,右侧颞叶痫样放电27例,双侧颞叶痫样放电18例,6例过度换气诱发试验出现癫痫波。临床症状中以强直-阵挛发作占TLE最多;TLE患者的EEG癫痫波多分布于一侧或两侧颞区。结论颞区的癫痫波对诊断TLE有重要作用;VEEG对提高TLE的诊断和定位具有重要意义。

Objective To explore the features of temporal lobe epilepsy（TLE）′s clinical characters and electroencephalogram（EEG）.Methods Gave 86 patients who had already been diagnosed TLE an EEG and Video-EEG（VEEG） per capita,and analysed the outcomes according to their clinical symptoms.Results There were 39 normal EEG outcomes and 47 abnormal EEG outcomes among 86 patients,and there were 5 normal VEEG outcomes and 81 abnormal VEEG outcomes.There were 30 patients who had epileptiform discharge in their left temporal lobes,27 patients in their right temporal lobes,18 patients in their bilateral temporal lobes and 6 patients had epileptiform waves during excessive ventilation test.The most symptom was tonic clonic seizure in TLE;the epileptiform waves of patients with TLE were often distributed over unilateral or bilateral temporal lobes.Conclusion The epileptiform waves play an important role in the diagnosis of TLE;VEEG is an efficient method for the diagnosis and localization of TLE.
