膈肌破裂的CT诊断价值(附18例手术对比分析)

The diagnosis value of CT with diaphragmatic rupture:compared analysis of 18 cases with surgeries

目的探讨膈肌破裂的CT诊断价值。方法归纳18例膈肌破裂患者的CT表现与诊断,与手术作对比,分析两者的符合程度。结果大部分患者CT均显示膈肌中断、断端膈肌增厚等特征性征象,其中8例合并膈疝者CT均明确显示。CT对膈肌破裂诊断的敏感度为72%,特异度94%,准确度88%。结论 CT检查对膈肌破裂的诊断有重要帮助,应作为胸腹急诊患者术前检查的首选方法 。

Objective To study the diagnosis value of CT with diaphragmatic rupture.Methods We concluded the manifestations and diagnosis of CT on patients with diaphragmatic rupture,compared with surgeries,and analyze their consistence.Results On CT the characteristic signs such as diaphragmatic defect and thickened were displayed in majority of the cases,including 8 who were combination with diaphragmatic hernias.Sensitivity,specificity and accuracy for diagnosing diaphragmatic rupture separately were 72%,94% and 88%.Conclusion CT examination has important help for diagnosing diaphragmatic rupture,and should act as the first choice in the pre-surgery examinations on patients with acute thoracic and abdominal diseases.
