贲门周围血管离断术治疗门脉高压症急性上消化道大出血62例临床治疗体会

目的探讨贲门周围血管离断术治疗门脉高压症急性上消化道大出血的临床方法和疗效。方法对我院自2003年1月至2009年1月共收治门脉高压急性上消化道出血62例行贲门周围血管离断术的患者进行回顾性分析。结果62例患者手术后达到即刻止血。术后切口裂开4例,大出血并发休克1例,1例并发轻度肝性脑病,急诊手术死亡1例。随访1～5年,再出血8例,其余患者肝功能改善。结论贲门周围血管离断术治疗门脉高压症急性上消化道大出血手术止血效果确切,术后并发症少,肝功能有明显改善,再出血机率低,死亡率低,可广泛适用。

Objective Of cardiac peripheral vascular disconnection in the treatment of portal hypertension with acute upper gastrointestinal bleeding in the clinical method and effect of.Methods In our hospital from January 2003 to January 2009 with portal hypertension were treated 62 acute upper gastrointestinal bleeding from broken blood vessels around a routine cardiac surgery patients were analyzed retrospectively.Results 62 patients immediately after surgery to stop bleeding.Incision dehiscence in 4 cases,massive hemorrhage complicated by shock in 1 case,1 case complicated by mild hepatic encephalopathy,emergency surgery and 1 died.Follow-up of 1 to 5 years,8 cases of rebleeding and the rest to improve liver function in patients.Conclusion Cardia peripheral vascular disconnection in the treatment of portal hypertension with acute upper gastrointestinal bleeding surgical hemostasis precise,less postoperative complications,liver function improved significantly lower risk of rebleeding,mortality is low,can be widely used.
