超选双侧子宫动脉栓塞治疗产后大出血

Treating postpartum massive hemorrhage with uterine arterial embolization

目的探讨子宫动脉栓塞术治疗产后大出血的临床应用价值。方法对22例产后大出血患者，行股动脉穿刺髂内动脉造影，明确出血部位后，超选插管，根据出血血管情况，经导管注入明胶海绵或（和）普通弹簧钢圈等栓塞双侧子宫动脉，以达到止血目的。结果22例产后大出血患者全部栓塞成功，其中包括3例凝血功能不全患者。随访1月～2年，无一例大出血现象，复查子宫附件未见明显异常。结论超选择性双侧子宫动脉栓塞术用于治疗产后大出血，迅速、安全、有效，无严重并发症，并可保留子宫，该技术应得到广泛应用。

Objective To Investigate the clinic value of uterial artery embolization for treating postpartum massive hemorrhage. Meth- ods 22 cases of postpartum massive hemorrhage underwent femoral artery puncture by seldinger＇s technique. DSA angiography showed the posi- tion of hemorrhagic artery. All cases of postpartum hemorrhage were performed arteries embolization with gelfoam or/and coil on the basis of hi- bateral uterial artery. Results The embolization succeed in all cases including 3 cases with coagulation disorders. All cases were follow - up visi- ted 1 month to 2 years, no rehaemorrhagia was observed. Conclusion UAE characterized by instant stanching, certain curative effect, minor serious complications , and uterine saving etc. It is a more effective way for the treatment of postpartum massive hemorrhage, this technology should be widely adopted.
