非霍奇金淋巴瘤中CD40、CD40L的表达及意义

摘要 目的探讨非霍奇金淋巴瘤(NHL)中CD40、CD40L表达与NHL预后间的关系及意义。方法免疫组织化学法检测50例非霍奇金淋巴瘤、22例淋巴结反应性增生组织中CD40、CD40L的表达。结果 (1)NHL中CD40过度阳性率(66.0%)显著高于淋巴结反应性增生(36.4%),P<0.05;NHL中CD40L过度阳性率(20.0%)显著低于淋巴结反应性增生(63.6%),P<0.05。(2)NHL中CD40与CD40L表达呈负相关,r=-0.486,P<0.05。(3)NHL患者CD40、CD40L的过度阳性率与恶性程度、结外浸润及临床分期均显著相关,P<0.05。结论 (1)CD40、CD40L过度阳性率与恶性程度、结外浸润及临床分期密切相关,其可能与NHL侵袭性及预后有关。(2)NHL中CD40表达的增高、CD40L表达的减少可能是影响其发病的因素之一。 Objective To study the relationship between the expressions of CD40,CD40L in the tissues of non-hodgkin lymphoma (NHL) and prognosis,clinical implication of patients with NHL.Methods The expression of CD40,CD40L in 50 NHL tissues and 20 reactive hyperplasia of lymph node tissues were determined by immunohistochemical method.Results (1)CD40 positive overexpression rate in NHL samples (66,0%) was higher than that of in reactive hyperplasia of lymph node samples (36.4%) (P0.05),CD40L positive overexpression rate in NHL samples (20.0%) was lower than that of in reactive hyperplasia of lymph node samples (63.6%) (P0.05). (2) There were negative correlations between CD40 and CD40L expressions in those(r=-0.486,P0.05).(3) There were significant correlations between the positive overexpression rates of CD40,CD40L,and malignant degree,infiltration beside lymph node organ and clinical stages in NHL patients. Conclusion (1) There are close correlations between CD40,CD40L and malignant degree,infiltration beside lymph node organ and clinical stages,which can likely be correlative in judging the NHL prognosis and infestation.(2)High CD40 expressions and low CD40 L expressions may probably be one of etiologic factors of NHL.
