困难型腹腔镜阑尾切除的治疗体会

EXPERIENCE IN TREATMENT OF DIFFICULT LAPAROSCOPIC APPENDECTOMY

目的探讨困难型腹腔镜阑尾切除术的手术技巧。方法回顾分析我院2005年5月～2009年12月用腹腔镜治疗50例困难型阑尾的临床资料结果手术困难原因:阑尾暴露困难、阑尾根部处理困难、阑尾系膜处理困难。除1例异位阑尾改开腹外,其余所有病例均在腹腔镜下完成。结论困难型阑尾在腹腔镜下切除是安全可行,不增加手术并发症,具有腹腔镜手术的优点。

Objective： To explore the operation skill of difficult laparoscopic appendectomy. Methods： Clinical data of 50 patients undergone difficult laparoscopic appendectomy from May 2005 to November 2009 were retrospectively analyzed. Results： The factors associated with difficult laparoscopic appendectomy included the difficulty of exposure, the difficult management of appendix root and mesoappendix. All cases were accomplished under laparoscope, except one ectopic appendicitis was converted to open surgery, Conclusion： Difficult laparoscopic appendectomy is safe,feasible and without increased complications .
