合并糖尿病的白内障患者超声乳化术后干眼分析

Clinical observation on dry eye after ultrasonic phacoemulsification in patients with cataract combined with diabetes

目的观察单纯老年性白内障与合并糖尿病的白内障患者行超声乳化术后的干眼情况。方法将行白内障超声乳化术患者202例(225只眼)分为A、B两组,A组为单纯老年性白内障患者110例(127只眼),B组为合并糖尿病的老年性白内障患者92例(98只眼),分别于术后1d、7d、1个月、3个月观察两组患者干眼症状、泪膜破裂时间、角膜荧光素染色以及基础泪液分泌试验等情况。结果 B组术后各时间点干眼症状明显重于A组,术后1、3个月泪膜破裂时间明显短于A组,术后各时间点角膜荧光素染色明显多于A组,差异均有统计学意义(P<0.01)。术后3个月,B组患者基础泪液分泌试验明显低于A组,差异有统计学意义(P<0.05)。结论相对于单纯老年性白内障患者,合并糖尿病的白内障患者超声乳化术后干眼发生更早,症状更明显,持续时间更长。

Objective To observe the difference of dry eye after ultrasonic phacoemulsification between the sim- ple age-related cataract and the cataract combined with diabetes. Methods Two hundred and two patients（225 eyes） who received ultrasonic phacoemulsification were divided into two groups： 110 patients （127 eyes） with simple age-relat- ed cataract in group A and 92 patients（98 eyes） with cataract combined with diabetes in group B. The subjective com- plains of dry eye, tear break-up time, corneal fluorescein staining and Schirmer I test in all patients were measured one day, seven days, one month and three months postoperatively respectively. Results Compared with group A, complains of dry eye and corneal fluorescein staining was more severe in group B with break-up time and Schirmer I test being re- duced more greatly. The difference between them was significant. Conclusions Compared with the simple age-related cataract, dry eye after ultrasonic phacoemulsification in cataract combined with diabetes occurred earlier which was more severe and lasted longer.
