结核性腹膜炎腹腔镜探查直视下诊断价值研究

The value research of tuberculous peritonitis laparoscopy diagnosis

目的探讨腹腔镜腹腔探查直视下诊断结核性腹膜炎的价值和意义。方法对新疆维吾尔自治区人民医院自2005年1月至2009年12月收治的65例怀疑结核性腹膜炎患者行全麻腹腔镜腹腔探查活检术。其中34例患者确诊为结核性腹膜炎。将本组患者腹腔镜探查直视下诊断结果与病检结果进行比较。结果本组手术均成功,腹腔镜直视诊断结核性腹膜炎敏感度88.2%,特异度90.3%。Youden指数(YI)=0.78。结论腹腔镜直视下诊断结核性腹膜炎确诊率高,安全可行。结合病理检查可做为金标准。

Objective To explore the value of tuberculous peritonitis laparoscopy diagnosis in direct vision.Methods A total of 65 patients with suspected of tuberculous peritonitis underwent diagnostic laparoscopic abdominal exploration and biopsy from January 2005 to December 2009,and 34 cases were diagnosed as tuberculous peritonitis.We compared the diagnosis between diagnostic laparoscopic exploration and biopsy.Results All the operations were successful in this group.Sensitivity of diagnostic laparoscopy for tuberculous peritonitis was 88.2%,and specificity was 90.3%.Youden index YI =0.78.Conclusions The laparoscopic diagnosis of tuberculous peritonitis in direct vision is safe and feasible,combining with histopathology,it can be used as the gold standard.
