高血压前期颈动脉内-中膜厚度与左心室结构的改变

Changes of the Intima- Media Thickness and Left Ventricular Structure of Prehypertension Carotid Artery

摘要 目的探讨高血压前期颈动脉内~中膜厚度(IMT),以及左心室结构和功能的改变。方法检测69例高血压前期患者的颈动脉IMT,以及左心室结构和功能,并与高血压前期组、血压正常组比较。结果颈动脉IMT:高血压组、高血压前期组颈动脉分叉、经总动脉处IMT均较正常血压组增厚,且差异有显著性意义(P〈0.01、〈0.05),高血压组颈内动脉IMT较正常血压组厚(P〈0.05);高血压前期组和血压正常组颈内动脉IMT无明显统计学差异(P〉0.05)。高血压前期组LVPwT、IVST、RwT、LVM、LVMI均高于血压正常组,有显著性差异(P均〈0.05);高血压前期组LVDD、E/A值、LVEF值与血压正常组相比无统计学差异(P〉0.05),而高血压组与血压正常组之间差异有显著性(P〈0.05、0.01、0.05)。结论高血压前期已经出现左心室结构的变化,左室壁厚度和左心室质量开始出现代偿性改变,但左心室功能尚无明显减退。 Objective To explore the changes of the intima -media thickness (IMT) and left ventricular structure and function of prehypertension carotid artery. Methods Intima - media thickness and left ventricular structure and function of carotid arteries of 69 patients were detected. Results Carotid arterial IMT: IMT of carotid artery bifurcation and the regions bypass the total artery of hypertension and prehypertion groups were significantly thicker than those with normal blood pressure group(P 〈0. 01 ,P 〈0. 05), the carotid artery IMT of hypertension group were thicker compared with normal blood pressure group ( P 〈0. 05 ), carotid artery IMT was no significant statistical difference between prehypertension and normal blood pressure group(P 〉 0.05 ). LVPWT, IVST, RMT, LVM, LVMI of prehypertensive group were higher than normal blood pressure group (P 〈 O. 05). LVDD, E/A value, LVEF values of prehyper- tensive group were of no significant difference compared with normal blood pressure, while there were sig- nificant different between hypertensive group and normal blood pressure groups(P 〈0. 05,0.01,0.05 ). Conclusion The left ventricular structural changes have emerged in prehypertensive stage, and left ven- trieular wall thickness and left ventricular mass have compensatory changes, while there is no obvious impaired left ventricular function.
