踝臂指数与彩色多普勒超声对糖尿病足患者下肢动脉病的预测价值 被引量:10

摘要 目的评价踝臂指数(ABI)与彩色多普勒超声(CDFI)对糖尿病足(diabetic foot,DF)患者下肢动脉病(LEAD)的诊断价值。方法 DF患者291例按照Wagner分级法分组,与60例同期住院非糖尿病患者(对照组)进行下肢血管彩色多普勒超声检查对照,并与踝臂指数对比观察。结果 DF组下肢动脉硬化各种病变发生率较对照组明显增高;DF组为广泛性多发血管段病变,对照组动脉硬化多为血管分叉处局灶性病变,差异有统计学意义(P<0.01),且胫前后动脉及足背动脉流速、RI、PI减低,与对照组比较差异有统计学意义(P<0.01)。在Wanger分级法中,ABI及CDFI对DF患者动脉硬化性病变均有较高的准确率,但ABI与CDFI对0~Ⅰ级病例动脉硬化性病变阳性率差异有统计学意义,与CDFI相比略低,对Ⅱ~Ⅴ级别病例两种检查手段阳性率无明显差别。结论 ABI及CDFI对评价DF血管病变均有良好的临床应用价值,两者联合检查,能更准确地预测DF患者动脉硬化性病变。
