摘要 目的探讨Survivin、血管内皮生长因子(VEGF)在甲状腺癌中的表达及其与预后的关系。方法采用免疫化学EliVison两步法检测70例甲状腺癌、14例甲状腺瘤及14例正常甲状腺组织中Survivin、VEGF的表达,并对甲状腺癌组织学分型、临床病理分期、病理类型及淋巴结转移等进行对比分析。结果70例甲状腺癌中Survivin、VEGF的阳性率分别为64%、66%,和正常甲状腺组织、腺瘤比较差异有统计学意义(P<0.05)。甲状腺癌中Survivin、VEGF的表达在不同组织学分型、病理类型、临床病理分期、淋巴结转移状态差异有统计学意义(P<0.05)。Survivin与VEGF的表达呈正相关(r=0.503,P<0.01);甲状腺癌中Survivin、VEGF表达阳性组和阴性组生存率差异有统计学意义(P<0.05),表达阳性组预后差。结论Survivin、VEGF在甲状腺癌的发生、发展中起着重要作用,二者联合检测可为甲状腺癌恶性程度和预后的判断提供有效证据。 Objective To study the expressions of Survivin and VEGF in thyroid carcinoma and their prognostic significance. Methods Expressions of Survivin and VEGF were detected in 70 cases of thyroid carcinoma, 14 of thyroid adenoma and 14 of normal thyroid tissues by EliVison immunohistochemistry, and meanwhile, compared with histology, TNM staging, pathology and lymph node metastasis of thyroid carcinoma. Results Survivin and VEGF were expressed in 64% and 66%, respectively, of the 70 cases with thyroid carcinoma, with significant difference compared with those in thyroid adenomas and normal tissues (P〈0.05). Expressions of Survivin and VEGF varied with histology, TNM stages, pathology and lymph node metastasis (P〈0.05). Expressions of Survivin and VEGF correlated positively with each other (r=0.503, P〈0.01). For patients with thyroid carcinoma, the survival rate differed statistically between those tested negative and positive for Survivin or VEGR (P〈0.05), with the worst prognosis found in patients who tested positive for Survivin and VEGF. Conclusion Survivin and VEGF may be playing an important role in carcinogenesis and progression of thyroid carcinoma. Combined detection can provide valuable evidence for estimation of malignancy and prognosis in thyroid carcinoma.
