氯胺酮对小鼠烧伤模型炎症细胞因子表达的影响

摘要 目的观察氯胺酮对热烧伤小鼠的治疗效果及对炎症细胞因子表达的影响。方法将40只小鼠按随机数字表法分为4组,每组10只,热烧伤诱导小鼠Ⅲ度烧伤模型。氯胺酮Ⅰ组、Ⅱ组、Ⅲ组和生理盐水组,按治疗方案分别给予相应治疗。3d后观察各组小鼠存活率以确定疗效最佳组。另取60只小鼠按随机数字表法分为氯胺酮组和芬太尼组和生理盐水对照组,每组20只,建立烧伤模型。氯胺酮组按最佳方案治疗。采用酶联免疫吸附法(ELIA)检测2h、6h、12h、24h时血清IL-6及TNF-α浓度;采用逆转录聚合酶链(RT-PCR)检测小鼠肝脏、肺脏组织中HSP-70 mRNA水平。结果氯胺酮Ⅰ组、Ⅱ组、Ⅲ组及生理盐水组小鼠存活率比较差异有统计学意义(P<0.05),氯胺酮Ⅲ组小鼠存活率最高(70%,P<0.05),为最佳治疗组;小鼠在烧伤后2h、6h、12h、24h时血清IL-6和TNF-α水平均明显低于芬太尼组和对照组(P<0.01);烧伤后12h时氯胺酮组小鼠肝脏、肺组织中HSP-70 mRNA水平均明显高于芬太尼组和对照组(P<0.01)。结论小鼠热烧伤后迅速、足量、持续使用氯胺酮可减低炎症细胞因子水平,提高小鼠存活率。 Objective To investigate the efficacy of ketamine on the burned mice and the influence on the secretion of the inflammatory cytokines.Methods 40 mice were divided into four groups by means of random number table(n=10)and burned mice were induced by a routine way.The four groups were treated by three ways with different dosages of ketamine and NaCl(control)respectively,and 3 days later,determine best efficacy group by analyzing the survival rate of mice in each group.Another 60 mice were divided into three groups(ketamine group,fentinyl group and control group)by means of random number table,and subsequently burned models were induced;serum IL-6 and TNF-α were measured by Elisa after 2 hours,6 hours,12 hours,24 hours respectively,and HSP-70 mRNA level in liver and lung was detected by reverse-transcriptase polymerase chain reaction(RT-PCR)after 12 hours.Results The survival rate of mice in group Ⅲ was highest in our experiment(P0.05),of which the dosage was optimal to treat burned mice with ketamine(P0.05).Serum IL-6 and TNF-α level in ketamine group were markedly low than those of fentinyl group and control group(P0.01),and the expression of HSP-70 mRNA in liver and lung tissue were significantly higher than those of fentinyl group and control group(P0.01).Conclusion Ketamine may increase the survival rate of burned mice and reduce the level of serum inflammatory cytokines.
