抑郁障碍患者生活事件和防御机制与发病的相关性研究

Lift events and defense mechanism of depressive disorder patients

目的探讨抑郁障碍患者生活事件和防御机制与发病的相关性。方法将70例抑郁障碍患者设为研究组，80例健康成年人设为对照组，对两组采用生活事件量表和防御方式问卷进行测评分析。结果研究组生活事件量表总分及频数均显著高于对照组（P〈0．01）。防御方式问卷评分，不成熟防御，投射，被动攻击，见诸行动，幻想和躯体化因子分显著高于对照组（P〈0．01）；中间型防御，伴无能诊断和否认因子分显著高于对照组（P〈0．01），而回避因子分显著低于对照组（P〈0．05）；成熟防御，升华因子分显著低于对照组（P〈0．05或0．01）。结论抑郁障碍患者生活事件繁多，多采用不成熟防御机制和中间型防御机制，缺乏升华能力；而生活事件和不适当的心理防御机制在其抑郁障碍的发病过程中起着重要的作用。

Objective To explore the correlation between life events and defense mechanism of depressive disorder patients. Methods Seventy 70 depressive disorder patients were assigned to research group and 80 healthy adults to controlgroup, assessments were conducted with the Lift Events Scale（LES） and Defense Style Questionnaire （DSQ）. Results Compared with contore group, the depressive patients had higher scores and more frequency in the LES（P〈0.01） ; compared with contorl group, the depressive patients had higher scores of immature defense mechanism, project, passive aggression, no action, fantasy and somatization factors of DSQ（P〈0. 01）; The depressive patients had higher scores of neutral defense of defense mechanism of DSQ,especially in the deny and the all capacity with incapacity factors（P〈0.01）, but avoidance lower （P〈0.05）; The depressive group had lower scores than the control group in mature defensse mechanism of DSQ,especially in the subimation factors. Conlusion The depressive patients have more life events, they apply more immature and intermediate defense mechanism. Life event and inappropriate psychological defense mechanism pay important role to depression.
