右美托咪啶对瑞芬太尼复合丙泊酚靶控输注麻醉苏醒期的影响 被引量:22

摘要 目的 探讨右美托咪啶对瑞芬太尼复合丙泊酚靶控输注(T C I)全麻苏醒期的影响.方法 40例ASAⅠ~Ⅱ级、瑞芬太尼复合丙泊酚靶控输注全麻下行腹腔镜胆囊切除手术的患者随机分为右美托咪啶组(D)、生理盐水组(C),D组术中缓慢静滴有美托咪啶(0.1 mg加入100 ml生理盐水中),历时15 min C组输注100 ml生理盐水.手术结束后滞留30 min,观察患者苏醒恢复期的清醒时间、拔管时间、拔管后0 min和20 min的平均动脉压、心率及镇痛评分、呼吸抑制、躁动评分(RS)、镇静评分(RSS)、全麻后身体舒适度评分(BCS).结果 C组的镇痛评分显著低于D组(P〈0.05).清醒时间、拔管时间、拔管时、拔管后平均动脉压(MAP)和心率(HR)比较,D组优于C组,差异有显著性(P〈0.05).呼吸抑制发生率D组和C组相比差异无显著性(P〉0.05).躁动评分(RS)、镇静评分(RSS)、全麻后身体舒适度评分(BCS)比较,D组优于C组,差异有显著性(P〈0.05).结论 瑞芬太尼复合泊酚靶控输注(TCI)全麻下行腹腔镜胆囊切除手术中,右美托咪啶应用不但使苏醒期的镇痛效果好,预防苏醒期躁动发生,血流动力学稳定,而且并发症发生率低.
