双手微小切口与传统同轴超声乳化吸除术术后并发症的Meta分析 被引量:5

摘要 目的系统评价双手微小切口超声乳化吸除术(bimanual microincisional cataract surgery,B-MICS)与传统同轴超声乳化吸除术(conventional coaxial small-incision cataract surgery,C-SICS)的手术并发症。方法计算机检索Medline、西文医学期刊文献数据库、OVID、中国生物医学文献数据库和中国知网,结合手工检索的方式,对纳入的有关B-MICS和C-SICS临床疗效的相关文献进行Meta分析,以比值比(odds ratio,OR)判定B-MICS和C-SICS的手术并发症的发生率,应用Review Manager 4.2软件计算纳入文献的异质性,根据结果选择固定效应模型或随机效应模型进行Meta分析,并对资料进行敏感性分析。结果按照文献的纳入与排除标准,经筛选后纳入Meta分析的文献共有10篇。手术并发症发生率的合并OR值为0.48(95%CI为0.27~0.85),差异有统计学意义(P<0.05),敏感性分析所得结果与之基本一致,合并0R值为0.45(95%CI为0.24~0.84),差异有统计学意义(P<0.05)。结论现有资料表明B-MICS可以有效减少手术并发症的发生。 Objective To evaluate the postoperative complications after bimanual microincisional cataract surgery(B-MICS) vs conventional coaxial small incision cataract surgery (C-SICS). Methods A systematic review related with B-MICS and C-SICS was conducted by a computer search in Medline, EMCC, OVID, CBM, CNKI and a supplement of mannual search. The complications were evaluated according to odds ratio (OR) between B-MICS and C-SICS. The software of Review Manager 4.2 was used to analyze the heterogeneity, on the basis of the result, to select fixed effort model or random effort model to carry out the meta-analysis. Then sensitivity analysis was performed. Results Through strictly including and excluding the relevant articals from the resource, a total of 10 trials were included in the meta-analysis. Summary OR of the meta-analysis showed a significant reduction of complications in B-MICS group( OR = 0.48, 95% CI : 0.27 - 0.85, P 〈 0.05 ), and the sensitivity analysis results were similar to the original results ( OR = 0. 45, 95% CI: 0. 24 - 0.84, P 〈 0.05 ). Conclusion The data available show that B-MICS can effectively reduce the incidence of complications after the operation.
