四种监测方法在胎儿宫内窘迫诊断中的比较 被引量:6

摘要 目的探讨无应激试验(NST)、脐动脉血流速度(S/D)值、羊水指数(AFI)、妊娠特异性β1糖蛋白(SP1)筛查胎儿宫内窘迫的临床价值。方法对480例孕34~43周孕妇随机分为4组,A组:NST监测组,B组:AFI监测组,C组:S/D值监测组,D组:SP1值监测组,每组120例。分别在产前、产时行NST、S/D值、AFI、SP1的测定,产后追踪分析其胎儿宫内窘迫发生率及胎儿预后,比较各种监测方法的符合率。结果 B组与A组阳性病例在新生儿1 min Apgar评分、羊水污染、脐血血气分析方面差异无统计学意义(P>0.05),C、D组与A组阳性病例在新生儿1 min Apgar评分、羊水污染、脐血血气分析方面差异有统计学意义(P<0.05)。B组与A组在假阳性率、假阴性率及符合率方面差异无统计学意义(P>0.05),C、D组与A组在假阳性率、假阴性率及符合率方面差异有统计学意义(P<0.05)。结论在胎儿宫内窘迫的诊断中S/D值、SP1的测定优于NST、AFI,但是4种方法都有不同程度的假阴性和假阳性。
