术中腹腔内温热灌注化疗综合治疗中晚期大肠癌的临床价值

Combined therapy of surgery and intraperitoneal hyperthermic perfusion chemotherapy for advanced colorectal cancer

目的探讨术中腹腔内温热灌注（ＩＰＨＰ）化疗在中晚期大肠癌中的临床价值。方法１２０例中晚期大肠癌病人随机分成Ａ，Ｂ两组。Ａ组６８例行手术结合腹腔内温热灌注化疗；Ｂ组５２例行单纯手术治疗。观察患者术中、术后临床表现，实验室检查和组织病理学变化，并进行随访分析。结果两组术中、术后临床表现无显著差异，Ａ组组织病理学改变明显，随访Ａ组疗效优于Ｂ组。结论术中ＩＰＨＰ化疗安全可行，对治疗大肠癌的腹膜转移效果显著，是控制中晚期大肠癌术后复发的一种有效方法。

Objective To determine the clinical value of intraperitoneal hyperthermic perfusion (IPHP) chemotherapy in preventing postoperative recurrence of advanced colorectal cancer. Methods One hundred and twenty patients with advanced colorectal cancer were randomly divided into 2 groups: Group 1, 68 cases were treated by IPHP chemotherapy; control group, 52 cases were only by operation. Clinic manifestation, histopathology and prognosis of Group 1 and Group 2 were compared. Results The clinical manifestation has no significant difference between two groups in operation and post operation periods, but hispathology has an encouraging change in Group 1. Follow up survey showed that the effective of therapy group was better than that in control group. Conclutions Surgical operation combined with IPHP chemoterapy is safe, applicable and effective in preventing post operative recurrence in advanced colorectal cancer, especially in the treatment of peritoneal metastasis.
