中文版32项轻躁狂症状清单在双相Ⅱ型障碍患者中的应用

The Chinese version of the 32-item hypomania checklist in patients with bipolar Ⅱ disorder

目的:研究中文版32项轻躁狂症状清单(32-item hypomania checklist,HCL-32)在双相Ⅱ型障碍患者中应用的效度、信度。方法:对69例双相II型障碍患者应用HCL-32进行测评,其中有26例(37.7%)患者在8～14d后重测。结果:经相关分析及Kruskal-Wallis检验,双相II型障碍患者在接受测评时的心境状态对HCL-32得分的影响无统计学意义(P分别为0.48、0.23)。因子分析显示前2个因子的特征值较佳,分别为6.19和3.57,前2个因子对总方差的累积贡献率为30.5%。内部一致性信度分析HCL-32全量表Cronbach'salpha值为0.84,因子I和因子II分别为0.85和0.66。全量表重测相关系数为0.51(P=0.007)。前后两次测评中,32项条目重测一致率为53.8%～96.2%。患者的HCL-32得分范围为3～26分,HCL-32总分≥14的患者有46例(66.7%)。量表分均值为(15.26±5.91)分。32项条目阳性回答率为7.2%～82.6%。结论:HCL-32中文版在双相II型障碍患者中初步试用,其效度、内部一致性信度尚满意,但重测信度偏低。

Objective：To investigate the reliability and validity of application of the chinese version 32-item hypomania checklist（HCL-32）in patients with bipolar Ⅱ disorder.Method：The Chinese version of HCL-32 were tested in 69 patients with bipolar Ⅱ disorder,and 26（37.7%）of them retested after 8 to 14 days.Results：There were no statistical associations between HCL-32 total scores and the subjects＇current mental state rating by the spearman correlation analysis（P=0.48）.Also the mean total HCL-32 scores did not differ between sub-groups defined by the current mental state by the Kruskal-Wallis analysis（P=0.23）.A two-factor solution was preferred by the factors analysis.The eigenvalues of the two factors were 6.19 and 3.57 respectively;the two factors together accounted for 30.5% of the total variance.The Cronbach＇s alpha of the total scale was 0.84;and of the first and second factor 0.85 and 0.66 respectively.The test-retest reliability of the whole scale was 0.51（P=0.007）.The rate of unanimous answer of 32 items in the test and retest ranged from 53.8% to 96.2%.Patients＇ total HCL-32 scores ranged from 3 to 26.There were 46 patients（66.7%）with scores higher than or equal to 14.The mean score of the patients was（15.26±5.91）.The frequency of positive response of 32 items ranged from 7.2% to 82.6% in the study.Conclusion：The chinese version of the HCL-32 of had good validity and internal consistency reliability in patients with bipolar II disorder,but the test-retest reliability was slightly low.
