拓宽视野成像新技术——全景超声成像及其临床应用 被引量:10

摘要 为了增加实时超声 的视野,一项全新的超声拓宽视野成像技术(ultrasound ex-tended-field of view imaging,又称Sie-scape imaging)己由Weng等于1996年推出。它利用先进的计算机图像处理程序,使传统的实时超声视野通过普通探头的自由移动(超声扫查),迅速拓宽成一幅与CT或MRI相似的现代全景超声图像(panoramic ultra-sound imaging,PUI)。为了探讨现代PUI技术的临床应用价值及其发展前景,我们于1997年8月开始引进Siemens公司新近推出的SONOLINE Elegra全数字超声诊断仪。该仪器除可常规进行实时灰阶和彩色多普勒超声诊断外,同时兼有现代全景超声成像的功能。截至今年9月30日,我们已经积累342例PUI检查经验并提出了临床应用初步报告。我们的研究表明,PUI对于范围宽广的肢体和躯干软组织如胸腹壁的正常结构和病变的断层显示效果最好,它显著优于实时超声;PUI在腹部多种器官和妇产科方面的应用也具有很大的潜力;即使在检查甲状腺、乳房、阴囊和阴茎等小器官方面,也能不同程度地弥补常现实时超声的不足。我们认为PUI具有广泛的临床应用价值,它有利于客观地显示和识别解剖学结构,明确病变的范围及其与邻近组织器官的关系。PUI技术与实时灰阶和彩色多普勒超声相结合,将使现代超声影像技术更加完善。 A new imaging technology named ultrasound extended -field -of -view imaging(Sie -scape imaging) was developed by Weng and his colleagues in 1996 in order to overcome the longstanding limitation of real -time ultrasound scanner only offering cross -sectional image with very narrow field of vievv. Using this sophisticated imaging technology by some advanced computer pro-cedures, a panoramic ultrasound imaging (PUI) somewhat like CT or MRI can be easily completed while a conventional real -time transducer being moved by free hand. To evaluate the clinical usefulness and prospect of modern PUI, a full digital ultrasound scanner SONOLINE Elegra developed by Siemens has been used from Aug. 1997 to Sep. 1998. This lecture was based on our clinical experience of more than 300 cases obtained during this period. We found that modern PUI could be applied to visualize vari-ous normai and abnormal soft-tissue struc- tures at the most parts of the body, especial-ly suitable to the upper and lower extremi-ties, trunk including the chest and abdominal walls, back and bottom; it also has potential usefulness for demonstrating many viscera and their lesions in different abdominal organs and in obstetrics and gynecology as well. PUI may even be used to clearly show the small parts and their lesions such as thy-roid, breast, scrotum etc. We concluded that PUI features wide cross-sectional views of the body, it is a good complement to conventional real-time ultrasound(gray-scale and color Doppler) in clinical applica-tion.
