卡孕栓用于初孕妇女无痛人工流产术的临床分析

Clinical analysis on application of carboprost to painless artificial abortion in primigravida

目的:探讨卡孕栓用于初孕妇女无痛人工流产术(简称人流)的临床效果及并发症发生情况。方法:选择门诊自愿要求实施无痛人流术的初孕妇女300例,随机分为两组。观察组150例,术前1h含服卡孕栓0.5mg后进行无痛人流术;对照组150例,术前不含服卡孕栓,常规施行手术。观察两组孕妇宫颈口扩张、镇痛效果、麻醉药物用量、手术时间、苏醒时间、术中出血量以及手术并发症情况。结果:观察组宫颈扩张程度、镇痛效果、手术时间、苏醒时间等指标结果均明显优于对照组(P<0.01)。手术并发症方面,观察组宫颈(宫腔)粘连、子宫穿孔、月经失调的发生明显少于对照组(P<0.01),漏吸的发生两组差异无显著性(P>0.05)。结论:无痛人工流产术前1h含服卡孕栓能有效扩张宫颈,增强麻醉效果,明显减少麻醉药物用量,缩短手术时间、麻醉苏醒时间、减少术中出血、降低手术并发症,是一种比较理想的终止初孕妇女早孕的方法。

Objective：To investigate the clinical efficacy and complications of carbprost in painless artificial abortion in primigravida.Methods：300 primigravidas voluntarily requiring for implementing painless artificial abortion in outpatient department were selected and randomly divided into two groups.The observation group（150 cases） received painless artificial abortion after 1h of sublingual administration carboprost 0.5mg,the control group（150 cases） was performed the conventional operation without preoperative sublingual carboprost.Cervical dilation,analgesic effect,dosage of narcotic drugs,surgery time,recovery time,intraoperative blood loss and operative complications were observed in the two groups.Results：In the indexes such as cervical dilation degree,analgesic effect,surgery time and recovery time,the observation group was superior to the control group（P 0.01）.In complications,cervical（uterine） adhesions,uterine perforation and occurrence of menstrual disorders in the observation group were significantly lower than those in the control group（P0.01）.The occurrence of leaky suction in the two groups had no significant difference（P0.05）.Conclusion：Sublingual carboprost 0.5mg at preoperative 1h in painless artificial abortion can effectively dilate the cervix,increase the anesthetic effect,obviously reduce the dosage of narcotics,shorten the surgery time and recovery time,reduce intraoperative blood loss and complications,which is an ideal way to terminate early pregnancy in primigravida.
