输尿管巢状亚型尿路上皮癌-2例报告及文献复习

目的：探讨巢状变异型尿路上皮癌的临床病理和免疫组化特征。方法：对2例具有特殊临床病理特征的榆尿管尿路上皮癌作光镜和免疫组化染色观察，结合临床资料进行分析。结果：巢状变异型尿路上皮癌的特殊组织学表现为癌细胞呈巢状，巢中央可有小腺腔形成，细胞棱异型性小，形态类似Brunn细胞巢，但其具有高度侵袭性，常浸润肌层。结论：巢状变异型尿路上皮癌为高级别浸润性尿路上皮癌，常侵袭固有肌层及血管，预后差。其良性样的形态学极易导致误诊误治。浸润血管、肌层和特殊的免疫组化表达可为其鉴别诊断提供了依据。

Objective ： To discussion the clinicopathologicai and immunohistopathologic of Nested variant of urothelial carcinoma. Method： we analyzed 2 cases of the special clinicopathologic features ureteroscope urothelial carcinoma with microscopy and Immunohistochemistry staining, combined with clinical data. Results： Nested variant of urothetial carcinoma have a bland morphology and the neoplastic cells show little atypia, and they are arranged in nests that mimic cystitis cystica or Brunn＇s nests, leading to a possible underrecognition of this tumorbut it has the aggressive behavior and invasion of the muscularis. Conclusiont In summary, nested variant of urothelial carcinoma is a deceptively bland--appearing neoplasm with potential of deep invasion and metastases that should not go unrecognized.
