基于网络数据库的社区易误漏诊疾病诊断三联征的研究

Diagnostic Triad Based on Web Database for Easily Misdiagnosed or Missed Diagnosed Diseases in Community

目的挖掘网络数据库中的临床信息,用3个关键的症状、体征组合对社区易误漏诊疾病进行辅助诊断。方法收集近5年中国典型病例大全和中国知网数据库(CNKI)中常见易误漏诊疾病的完整病例资料12种,共1 562例,以疾病的临床症状、体征为自变量,采用描述性研究的方法筛选出疾病的诊断三联征,将选出来的三联征与该疾病在国外已有的诊断三联征进行比较。结果基于网络数据库得到12种疾病的36项主要症状体征,与目前国外已有对应疾病的诊断三联征相一致的有22项,一致率为61.1%。结论基于网络数据库的疾病诊断三联征研究具有资料容易获得、高效、可操作性强的特点,有进一步研究的价值。

Objective To explore the clinical information of web database to set up a diagnostic triad consisting of three key clinical symptoms and signs for assistant diagnosis of the diseases that are likely to be misdiagnosed and missed diagnosed.Methods The data of 1 562 patients with complete information of 12 kinds of common diseases that were likely to be misdiagnosed and missed diagnosed were collected from Complete Book of Typical Case Reports in China and CNKI.The clinical symptoms and signs as variables were analyzed by descriptive methods to screen the diagnostic triad for the 12 kinds of diseases,and then the screened triad was compared with that being used abroad.Results Totally 36 items of key symptoms and signs of the 12 kinds of diseases were screened according to the web database,among them 22 were consistent with those diagnostic triads of the corresponding diseases being used abroad,with consistency rate of 61.1%.Conclusion The diagnostic triad based on web database is characterized by readily available,highly efficient and operable,it is worth studying further.
