早期肠内营养加止血散在急性重症胰腺炎中应用

The application of Zhixue-powder with early enteral nutrition in the therapy for severe acute pancreatitis(SAP)

目的观察止血散加早期肠内营养在治疗急性重症胰腺炎(SAP)中疗效。方法 63例SAP患者随机分为治疗组(30例)和对照组(33例)。所有病例均予禁食、胃肠减压、补液、纠正酸碱电解质紊乱,生长抑素,抗菌素,质子泵抑制剂及对症治疗加肠外营养。治疗组在胃肠减压管注入止血散同时,在肠麻痹改善情况下行早期肠道内营养辅助治疗。结果治疗组与对照组相比较腹胀痛缓解时间有统计学显著性差异(P<0.05),治愈率分别为96.6%和78.8%,死亡率分别为3.3%和26.9%,胰周感染发生率分别为6.6%和27.3%。住院时间分别为15d和21d,住院统计学分析有显著差异(P<0.05)。结论止血散加早期肠道内营养能提高SAP治愈率,减少并发症,缩短住院时间,降低死亡率。

Objective To observe the curative effect of Zhixue powder with early enteral nutrition in the therapy for severe acute pancreatitis.Methods 63 SAP patients were randomly divided into the therapy group（30 cases）and the control group（33 cases）.All the patients had abrosia and gastrointestinal decompression,and received fluid infusion.The electrolyte disturbances were corrected.The patients also were treated with growth hormone release inhibiting hormone,antibiotic drug,proton pump inhibitor,symptomatic treatment and parenteral alimentation.Results Compared with the control group,the therapy group patients’ abdominal relieved fast,and it has statistical significance（P〈0.01）.The cure rates were 96.6%and 78.8%,death rates were 3.3%and 26.9%,peri-pancreas infection rates were 6.6%and 27.3%.length of stay were 15 d and 21 d.they all have statistical significance（P〈0.05）.Conclusion The treatment of Zhixue powder with early enteral nutrition is able to raise the cure rate of SAP,decrease the complication,shorten the length of stay and cut down the death rate.
