尿微量蛋白在早期诊断儿童过敏性紫癜肾脏损害中的应用 被引量:20

摘要 目的:探讨尿微量蛋白中的尿微量白蛋白(UAlb)和β2-微球蛋白(β2-MG)的检测对早期诊断儿童过敏性紫癜肾脏损害的临床意义。方法:选取本院儿科住院的患有过敏性紫癜的患儿共72例作为观察组(分为HSP组HSPN组)。对照组选取体检健康的儿童50例。观察组晨起用尿干化学试带法测定尿常规,用免疫比浊法测定β2-微球蛋白含量和尿微量白蛋白含量,再取24h尿液用甲苯防腐,取10ml离心,之后用终点法测24h尿总蛋白定量的值。对照组标本的留取和检测项目、方法与观察组相同,记录实验结果,对数据进行统计学分析。结果:对照组与HSP组和HSPN组的尿微量蛋白测定的比较有统计学意义(P<0.01),而尿蛋白定量HSP组对照组比较差异无统计学意义(P>0.05),同时尿常规和尿蛋白定量在异常检出率方面明显低于尿微量白蛋白和β2-微球蛋白。结论:尿微量蛋白(UAlb和β2-MG)的检测可作为早期检测儿童过敏性紫癜肾脏损伤的灵敏性指标。 Objective:To explore the clinical significance of urine micro albumin(UAlb) and β2-microballoons protein(β2-MG)examination at the early kidney damage of diagnosis of child allergic purpura.Methods:72 cases of HSP patients were the observation group.50 cases of health child were the control group.The observation group early morning appoints the urine to do chemistry to try the belt law determination routine urine test.The immunity turbidimetric method determination β2-microballoons protein content and the urine micro albumin content.Take 24 hour urine again with the toluene anticorrosion,and take the 10ml offcenter,afterward measures 24 hours urine total protein quota with the end-point method the value.The control group was the same with the observation group about examination.Record experimental result,and carry on statistics analysis to the data.Results:There was statistics the normal control significance(P0.05)to compare group urine micro protein with the HSP group and the HSPN group.The routine urine test and the urine protein quota is lower than the urine micro albumin and β2-microballoons protein obviously in the unusual detection rate.Conclusion:The examination of urine micro protein(UAlb and β2-MG)may examine the sensitive target which as the early time the child allergic purpura kidney damages.
