前列腺汽化电切术后出血原因及治疗对策分析

Analysis of the reasons and treatments of bleeding after tranurethral vaporization of the prostate

目的分析前列腺汽化电切术后出血的原因探讨,减少术后出血提高临床疗效的措施。方法前列腺汽化电切术后出血的原因包括患者自身疾病、术中电切与止血技术、以及术后腹压升高等多种。回顾分析2001年10月至2008年10月前列腺汽化电切术后出血46例的临床资料,根据术前、术中、术后的不同情况,采用术前降压、加压加快冲洗速度,再次镜下止血,持续牵引尿管等方法处理,观察治疗效果。结果根据不同的情况采取不同的治疗措施,均达到有效止血。结论前列腺汽化电切术后出血的原因较多,针对不同原因区别处理是提高手术安全,减少出血的有效措施。

Objective To explore the reasons and treatments of postoperative hemorrhage of transurethral vaporization resection of the prostate（TUVP）.Methods Forty-six patients with postoperative hemorrhagic after TUVP from October 2001 to October 2008 were analyzed retrospectively.The reasons of postoperative hemorrhage of TUVP include the accomplied diseases of the patients,the skills of resection and coagulation during operation,the sudden high abdomen pressure,et al.The good hemostatic effect can be achieved by controling the blood pressure before the operation,speeding up irrigation in the operation and sustained traction catheter-ization after the operation.Results According to the different condition of the patients,they received diffifent treatment.All the pa-tients got stable hemostatic effect.Conclusions Many reasons can cause postoperative hemorrhage after transurethral vaporization re-section of the prostate.Individual treatment can be done according to different conditions before,in and after the TUVP to improve surgical safety and reduce the hemorrhage.
