彩超引导下肉毒毒素注射治疗脑卒中后肌痉挛的临床疗效观察

目的探讨彩超引导下A型肉毒毒素注射治疗脑卒中后上下肢肌痉挛的临床疗效及操作可行性。方法 8例因脑卒中致上肢或下肢肌痉挛患者,在彩色多普勒超声仪引导下,进行痉挛肌肉内局部注射A型肉毒毒素。采用改良Ashworth评分(MAS)和改良Barthel指数评分(MBI)观察注射前、注射后4周肌张力及日常生活活动能力的变化。结果注射后4周MAS、MBI的改善均优于注射前(P<0.05)。结论彩超引导下的肉毒毒素注射治疗能明显改善脑卒中后肌痉挛,定位准确,值得推广。

Objective To investigate the feasibility and clinical effect of color doppler ultrasound guided BTXA injection in treating muscle spasm after stroke.Methods Eight patients with extremities muscle spasm after stroke were recruited in this clinical study.Under the guide of color doppler ultrasound,BTXA was injected into multi-points of muscles.The outcomes before injection and after BTXA injection four weeks were assessed by modified Ashworth scale（MAS） and modified Barthel index（MBI）.Results Compared the scores of MAS and MBI after 4 weeks with before treatment,there were significant differences statisti-cally（P0.05）.Conclusion Color doppler ultrasound guided BTXA injection technique is an effective and accurate positioning method in using BTXA to treat extremities muscle spasticity.
