102例冠状动脉造影无狭窄病变的胸痛患者的临床分析

目的回顾性分析在我院行冠状动脉造影无明显狭窄病变的102例胸痛患者,探讨相关的临床资料,以进一步提高对此类胸痛患者的认识。方法总结自2003年12月～2007年3月冠脉造影无明显狭窄病变的102例胸痛患者,进行临床分析和造影结果再认识。结果 102例胸痛患者经冠脉造影后均排除冠心病诊断,其最后的临床诊断多种多样,以心脏神经症居多,占47例(46.07%)。回顾此类胸痛患者的造影示内膜光滑,无局限性狭窄,但大部分病例的冠状动脉呈迂曲,扭结,有血流缓慢等现象。结论冠脉无明显狭窄病变而有冠脉迂曲的病人可出现典型劳累型心绞痛、冠状T波、运动试验阳性、采用以β受体阻滞剂为主的抗心绞痛治疗有效等表现,临床上酷似冠心病,冠状动脉造影对这类患者的鉴别有特殊意义。

Objective To disscuss the clinical informations of 102 cases without coronary stenosis defined with coronary angiography in order to emphasize the importance of coronary angiography on suspected chest-pain patients.Methods Collected 102 cases without coronary stenosis from Dec 2003 to Mar 2007,and make the clinical analysis.Results Their diagnosis were confirmed to divergent diseases and most of cases were cardiac nucrosis（46.07%）.Conclusion Many patients of coronary artery tortuosity without coronary stenosis may suffer from typical angina and to be diagnosed as coronary heart disease easily.Coronary angiography is necessary for identifying the chest pain patients from real coronary heart disease.
