Ultrasound Assessment of Perthes' Disease

目的 探讨Ｂ超检查对Ｐｅｒｔｈｅｓ 病的评价， 特别是软骨与软组织结构变化的评价， 为病情估计治疗选择及随访工作提供一定依据。方法 对３９ 例单侧Ｐｅｒｔｈｅｓ 病病例采用髋关节节前侧切面进行双侧对比检查， 同时随机抽取３０ 例正常髋作为对照。结果 Ｐｅｒｔｈｅｓ 病患髋存在关节腔扩大、关节囊及关节软骨增厚， 同时存在髂腰肌变薄及回声增强， 在Ｐｅｒｔｈｅｓ 病健髋有关节软骨增厚。Ｂ超分期与病程及骨盆平片Ｃａｔｔｅｒａｌｌ 分期存在正相关关系。结论 Ｂ超检查有利于病情估计， 为治疗提供一定参考， 同时可作为随访检查手段。

Objective The aim of this study is to examine the morphological changes of soft tissue in Perthes' disease under B mode ultrosonography. Methods The affected side of 39 patients with unilateral Perthes' disease was examined in contrast with the contralateral by anteriorlateral imaging and 30 normal served as control. Results The affected hip joint space was enlarged. Both the capsule and articular cartilage were thickened. The iliopsoas muscle was thinner than normal and hyper echoic. There was positive correlation between ultrasound staging of the disease and plain film classification by Catterall. Conclusions Ultrasound study is useful in assessing the pathological extent of the joint in patients with Perthes' disease. It also provides a mean of evaluation during follow up.
