糖尿病性白内障房水中细胞因子IGF-1、bFGF和IL-6的含量测定 被引量:9

Detection of IGF-1,bFGF and IL-6 levels in aqueous humor of diabetic cataract patients

摘要 目的检测糖尿病性白内障患者房水中胰岛素样生长因子-1(insulin-like growth factor-1,IGF-1)、碱性成纤维细胞生长因子(basic fibroblast growth factor,bFGF)和白介素-6(interleukin-6,IL-6)的含量,分析其与糖尿病性白内障患者相关性及临床意义。方法采用双抗体夹心酶联免疫吸附法测定71份白内障患者术前房水中IGF-1、bFGF-1和IL-6的含量。分为实验组:糖尿病性白内障患者房水38份,包括无糖尿病视网膜病变组(non-diabetic retinopathy,NDR)房水25份和伴有非增生性糖尿病视网膜病变组(non-proliferative diabetic retinopathy,NPDR)房水13份,对照组为年龄相关性白内障患者房水33份,以同样方法测定。结果实验组患者房水中IGF-1含量(7.47±5.03)ng·L-1和bFGF含量(7.47±7.08)ng·L-1均高于对照组(4.13±2.52)ng·L-1和(3.44±2.92)ng·L-1,差异有统计学意义(P<0.01或P<0.05);IL-6含量实验组与对照组比较,差异无统计学意义(P>0.05)。NDR组和NPDR组房水中IGF-1含量均高于对照组,差异有显著统计学意义(P<0.01);NDR组和NPDR组房水中bFGF含量分别为(7.44±6.59)ng·L-1和(7.52±8.22)ng·L-1,均高于对照组(P<0.05);3组间房水中IL-6含量比较,差异均无统计学意义(均为P>0.05);房水中IGF-1与bFGF之间、IGF-1与IL-6之间呈正相关性(r=0.409,P<0.01;r=0.259,P<0.05),但bFGF与IL-6之间无相关性(r=0.196,P>0.05)。结论糖尿病性白内障患者术前房水中bFGF、IGF-1含量的增加可能与高糖状态晶状体上皮细胞受损、凋亡及血-房水屏障破坏有关,房水中FGF、IGF-1和IL-6水平差异反映了不同细胞因子在不同发病阶段的参与作用,可作为治疗和预后的参考指标。 Objective To detect the levels of insulin-like growth factor-1(IGF-1),basic fibroblast growth factor(bFGF)and interleukin-6(IL-6)in aqueous humor of diabetic cataract patients,and to analyze their clinical significance and relation with diabetic cataract.Methods The concentrations of IGF-1,bFGF-1 and IL-6 in 71 samples of aqueous humor in diabetic cataract patients were measured by double antibody enzyme linked immunosorbent assay before operation.In experimental group,there were 38 samples of aqueous humor in diabetic cataract patients,including 25 samples of aqueous humor with non-diabetic retinopathy(NDR)and 13 samples of aqueous humor with non-proliferative diabetic retinopathy(NPDR).There were 33 samples of aqueous humor in patients with age-related cataract in control group,and detected by the same method.Results The IGF-1 and bFGF concentrations in aqueous humor of diabetic cataract patients were(7.47±5.03)ng·L-1 and(7.47±7.08)ng·L-1,which were higher than (4.13±2.52)ng·L-1 and (3.44±2.92)ng·L-1 in samples of aqueous humor in control group,there were statistical differences(P0.01 or P0.05).Concentration of IL-6 in experimental group had no difference with that in control group(P0.05).Concentrations of IGF-1 in aqueous humor of NDR group and NPDR group were higher than that in control group,there were significant statistical differences(both P0.01).Concentration of bFGF was (7.44±6.59)ng·L-1 in aqueous humor of NDR group higher than that in control group,the difference was significant(P0.01).Concentrations of bFGF in aqueous humor of NDR group and NPDR group were higher than that in control group,there were statistical differences(P0.01 or P0.05).No significant difference was found in concentration of IL-6 between each two groups(all P0.05).Positive correlations were found between bFGF and IGF-1,IGF-1 and IL-6(r=0.409,P0.01;r=0.259,P0.05),but no correlation was found between bFGF and IL-6(r=0.196,P0.05).Conclusions Increased level of bFGF and IGF-1 in aqueous humor of diabetic cataract patients may be related to injury and apoptosis of lens epithelial cells and damage of blood-aqueous barrier under high glucose condition.Differences of bFGF,IGF-1 and IL-6 levels in aqueous humor reflect the role and effect of different cytokines in different stages,which can be used as reference indicators for treatment and prognosis.
