摘要 目的探讨非穿刺探头引导下同轴活检针配合导向经皮肾脏穿刺活组织检查的应用价值。方法采用彩色多普勒超声诊断仪和普通探头,对45例肾脏疾病患者进行肾脏活组织检查,将同轴活检针预置于肾包膜处,在超声实时监控下,取出针芯,插入巴德穿刺枪进行穿刺活检。结果每次进针所获得的肾组织的长度2~15mm,平均(8.39±3.41)mm。光镜下每例所得的肾小球的数目为5~47个,平均(20.02±12.46)个,超声定位下均成功取出肾组织。2例术中出现穿刺点肾包膜下小血肿,其余均无并发症发生。结论非超声穿刺探头引导下同轴活检针穿刺肾脏,是一种简单、安全、准确的诊断方法 ,取材成功率高,并发症少,有利于在基层医院广泛开展。 Objective To explore the application value of coaxial biopsy needle combined with pereutaneous renal biopsy under ultrasound guidance without the use of puncture probe. Methods Kidney biopsy was performed on forty - five patients with renal disease by using color Doppler ultrasound and general probe. The coaxial biopsy needle was pre - placed adjacent to the renal capsule, Bard biopsy gun was inserted for renal biopsy specimen after removing the core needle under ultrasonic real time monitoring. Results The length of renal biopsy specimens was 2 mm-15 mm, the mean length was ( 8.39 ±3.41 ) mm,the number of renal glomeruli under microscope were 5 - 47 ,the mean number were (20.02±12.46). The renal tissue was dislodged successfully under ultrasonic localization. No complications occurred in all eases except small renal subcapsular hematoma in two cases. Conclusion Coaxial biopsy needle technique in renal biopsy without puncture probe under ultrasound guidance is a simple, safe and accurate diagnostic method. It has high successful rate and few complication, it is good to spread widely in primary hospitals.
