脊髓血管畸形二例临床分析

Analysis of the two patients of spinal cord vascular malformation

目的探讨脊髓血管畸形(SCVM)的临床表现、诊断及鉴别诊断。方法回顾性分析二例SCVM患者的临床资料。结果二例患者均为成年男性,隐袭起病,以运动障碍、感觉障碍及大小便障碍为主要症状,反复发作,进行性加重,曾均被误诊为"脊髓炎"。脊髓MRI可见多条蚯蚓状蜿蜒血管流空影,伴脊髓受压、缺血改变。结论脊髓血管畸形患者临床表现缺乏特异性,易与脊髓肿瘤、脱髓鞘病变及脊髓炎症相混淆,MRI及DSA检查有助于鉴别诊断。

Objective To discuss the clinical characteristic,diagnosis and differential diagnosis of spinal cord vascular malformation.Methods the clinical dates of two patients of spinal cord vascular malformation were reviewed.Results Both of the two patients were adult men,who went to bad ingradually.The symptoms were characterized by motor disability,sensory impedient and unabled to relieve themselves.They were once misdiagnosed as ＂myelitis＂.Magnetic resonance imaging found out low signal flow voids representing vessles,which was helpful to make certain of diagnosis.Conclusion Symptoms of spinal cord vascular malformation were not peculiar,so they should be differentiated from spinal tumor,demyelination and myelitis.Magnetic resonance imaging and digital subtraction angiography would be selected first.
