食管癌组织中α-SMA,CD34及VEGF的表达与其临床意义 被引量:1

摘要 目的 探讨食管癌组织中α-平滑肌肌动蛋白(α-SMA)、CD34、血管内皮生长因子(VEGF)蛋白表达的改变及与食管癌侵袭转移的关系,探讨三者在食管癌血管生成中的临床意义.方法 应用免疫组织化学方法检测40例食管癌组织及29例癌旁组织中纤维母细胞α-SMA,CD34和VEGF的表达,分析三者之间及与食管癌临床病理特征的关系.结果 40例食管癌组织α-SMA,CD34和VEGF的阳性表达与29例癌旁组织纤维母细胞中的阳性表达(82.50%vs 37.93%,χ2=14.454;32.50%vs 72.41%,χ2= 10.715;72.50%vs 20.69%,χ2=18.055;均P〈0.05);α-SMA和VEGF均与肿瘤浸润深度、临床分期和淋巴结转移有关(均P〈0.05),且两者的表达密切相关(r=0.453,P〈0.01).结论 食管癌组织中分布有大量的α-SMA表达阳性的肌纤维母细胞,α-SMA,CD34和VEGF的表达有一定的内在联系,三者联合可作为判断食管癌侵袭转移及评估预后的重要指标. Objective To study the expression changes of a-smooth muscle actin ( a-SMA) , CD34, vascular endothelial growth factor(VEGF) protein of esophageal carcinoma and the relationship between the invasion and metastasis of esophageal carcinoma. Analyze the correlation between a-SMA, CD34, VEGF and explore their mutual relations and clinical pathological significance in tumor angiogenesis of esophageal carcinoma. Methods The immunohistochemical method was used to detect fibroblast a-SMA, CD34 and VEGF expression of 40 cases of esophageal carcinoma tissues and 29 cases of adjacent tissues and analyze the relationship among the three and their relationship with the clinicopathological features of esophageal cancer. Results The positive expression of a-SMA, CD34, and VEGF in fibroblasts in 40 cases of esophageal cancer tissues and 29 cases of adjacent noncancerous tissues ( 82.50% vs 37.93% ,X2 = 14. 454 ;32.50% vs 72.41% ,X2 = 10. 715 ;72.50% vs20.69% ,X2 = 18. 055 ;both P 〈 0.05) ;a-SMA, and VEGF are related to depth of tumor invasion, clinical stage and lymph node metastasis ( all P 〈 0.05 ) , and the expression of the two are closely related ( r = 0. 453, P 〈 0. 01 ). Conclusion There is a large number of a-SMA-positive myofibroblasts in esophageal tissue indicating a certain internal relations among the expression of a-SMA, CD34 and VEGF; the three combined could be used as important indicators in invasion and metastasis of esophageal cancer and evaluation of prognosis.
