肾上腺间质组织肿瘤的CT和MRI表现

Manifestation of CT and MRI in patients with adrenal interstitial tissue tumors

目的分析５例罕见的肾上腺间质组织肿瘤的ＣＴ和ＭＲＩ表现。方法对５例经手术和病理证实的肾上腺间质组织肿瘤的ＣＴ和ＭＲＩ表现进行回顾性分析。其中右肾上腺神经纤维瘤２例，右肾上腺神经鞘瘤１例，左肾上腺纤维脂肪瘤１例，双侧肾上腺脂肪瘤１例。结果１例神经纤维瘤和１例神经鞘瘤有大的囊变区，１例神经纤维瘤显示多发性钙化和片状的增强。１例纤维脂肪瘤为Ｔ１ＷＩ低信号，Ｔ２ＷＩ稍高信号，注射钆剂后明显增强。１例双侧脂肪瘤为密度均匀、有薄而光滑的包膜的脂肪性肿块。结论除脂肪瘤外，其他肿瘤均不能与常见的腺瘤或嗜铬细胞瘤鉴别。

Objective To analyze the CT and MRI findings in five patients with adrenal interstitial tissue tumors. Methods All cases were verified with operation and histology. Two cases were neurofibromas; one was neurilemoma; all arising from the right adrenal. There was one left adrenal fibrolipoma; and the remaining one had bilateral adrenals lipoma. Results There was a large cystic degenerative area in a neurofibroma and a neurilemoma. One neurofibroma showed multiple calcification and patchy enhancement. One fibrolipoma exhibited low signal intensity on T 1 weighted image and slightly high signal intensity on T 2 weighted image, the signal intensity of the tumor increased obviously after Gd DTPA administration. The case with bilateral adrenal lipomas showed homogeneous fatty masses with a thin and smooth capsule. Conclusions It is impossible to distinguish these tumors, except lipoma, from common adrenal masses, such as adenoma or pheochromocytoma.
