胃肠道间质瘤的CT诊断

CT diagnosis for gastrointestinal stromal tumors

目的探讨胃肠道间质瘤的CT扫描表现和诊断价值。方法回顾性分析28例经手术病理和免疫组化确诊的胃肠道间质瘤,均行CT平扫和动态增强扫描。方法病灶发生在胃16例,十二指肠2例,空肠6例,回肠2例和直肠2例。其中18例为恶性,7例为交界性,3例为良性。CT扫描表现特征为:肿块向腔外或腔内生长,且较密实,较大肿块内可见囊变坏死区,增强扫描肿块均匀等密度者呈均匀中度或明显强化,尤以门脉期强化明显;肿块内坏死囊变者其实体部分强化,亦以门脉期强化明显。方法 CT平扫和动态增强扫描对胃肠道间质瘤的诊断具有重要意义。

Objective To analyze the CT image of gastrointestinal stromal tumors（ GIST） and it’s diagnostic value.Methods The clinical and image data of 28 cases with gastrointestinal stromal tumor confirmed by surgery and pathology and immunohistochemistry were analyzed retrospectively.Results 15 patients with lesions in the stomach, 2 cases in duodenum, 6 cases in jejunum, 2 cases in ileum, 2 cases in rectal and in 1 case in mesenteric. Of which 18 cases were malignant stromal tumors, 7 cases were borderline stromal tumor, and 3 cases were benign stromal tumor. CT scans show the common characteristics： tumor growth can be both inside and outside the cavity, with more dense, more visible cystic necrotic area in mass. Enhanced and uniform mass density scan showed moderate or marked enhancement, especially in the portal phase. Cystic changes in necrotic area often showed marked enhancement of tumor surrounding entities, also portal venous phase significantly.Conclusion CT is a major image techniques to diagnoze gastrointestinal stromal tumor.
