三维CT血管造影在神经外科术前评估中的初步应用经验

Preliminary experiences in preoperative evaluation of neurological disease with three dimensional CT angiography.

目的探讨应用三维ＣＴ血管造影（ＣＴＡ）在神经外科疾患术前诊断及评估中的作用。方法对４０例患者（２３例脑血管病、４例颈部血管性病变和１３例颅底或高血供脑瘤）进行了４３次ＣＴＡ和２６次数字减影血管造影（ＤＳＡ）检查。结果ＣＴＡ的诊断与ＤＳＡ结果相同并被手术证实。ＣＴＡ不仅发现８例动脉瘤和８例动静脉畸形，并且显示了这些病变的三维构造和毗邻的骨结构。ＣＴＡ准确地显示了颈动脉、椎动脉的狭窄并可显示颅内肿瘤与毗邻血管及颅骨的关系。结论ＣＴＡ具有简单、快速、无创和可靠等优点，适合于脑血管病、血供丰富的颅内肿瘤及颅底肿瘤的术前诊断和评估。

Objective To assess the usefulness of CT angiography(CTA) for diagnosis and preoperative evaluation of neurological diseases.Methods Fourty three times of CTA were performened in 40 patients,including 23 cases of intracranial cerebrovascular diseases,4 cases of extracranial cerebrovascular diseases and 13 cases of intracranial hypervascular tumours or skull base tumours.Twenty six of the 40 cases also underwent cerebral digital subtraction angiography(DSA).Results The results of CTA which were demostrated by operations showed very accuracy comparing with DSA.Eight intracranial aneurysms and 8 cerebral arteriovenous malformations were not only revealed by CTA, but also the three dimensional configuration of the vascular lesions and the surrounding bone structures were delinated.The stenosis of carotid artery and vetebral artery, the intracrarnal turnours with surrounding vessels and bone structures were displayed by CTA precisely.Conclusions
