摘要
目的:观察成骨蛋白-1(osteogenic protein-1,OP-1)对髓核抽吸术后椎间盘影像学改变的影响。方法:32只日本大耳白兔随机分成2周、4周、8周、12周4组,所有兔子用21号针头行L3/4椎间盘后外侧穿刺,抽吸出部分髓核组织,用微量注射器注射OP-125μl作为实验组(OP-1组),行L1/2椎间盘穿刺抽吸部分髓核组织并注射25μl生理盐水作为对照组(Saline组),术后2周、4周、8周、12周时按照分组取8只兔子行腰椎侧位X线摄片检查,测量施术椎间隙高度并计算椎间盘高度指数(DHI),行正中矢状位MRI检查,观察T2加权信号强度改变。结果:X线检查结果表明,OP-1组及Saline组术后2周、4周、8周、12周椎间盘高度呈降低趋势,但OP-1组较Saline组高度降低缓慢,各时间点两组DHI差异显著(P<0.05);MRI检查结果表明,OP-1组椎间盘术后2周、4周、8周、12周的髓核信号强度均较同期的Saline组椎间盘髓核信号强度高,二者Thompson评分结果差异显著(P<0.05)。结论:OP-1能延缓后外侧纤维环穿刺髓核抽吸术后兔椎间盘X线高度的降低和MRI T2加权信号强度的减低。
Objective:To investigate the radiographic effect of osteogenic protein-1(OP-1)on lumbar disc after nucleus pulposus aspiration in rabbits.Method:32 Japanese rabbits were randomly divided into 2 weeks,4 weeks,8 weeks and 12 weeks group.All the rabbits′posterior annulus fibrosus of L3/4 disc were stabbed by 21-gauge hypodermic needle,then some nucleus pulposus tissues were aspirated out.25μl OP-1 were injected into test discs using micro-injection syringe as test group(OP-1 group).While lumbar 1/2 disc were performed the procedure as lumbar 3/4 and 25μl saline were injected as blank control(Saline group).2 weeks,4 weeks,8 weeks and12 weeks after operation,lateral X-ray was performed on 8 rabbits and the disc height as well as disc height index(DHI)were measured,and the T2 weighted signal intensity of each group was measured under MRI.Result:Disc height decreased gradually with time passing in both OP-1 group and without significant difference between the two groups,and at each time point,the difference of DHI between two groups was significant(P0.05).MRI showed the nucleus pulposus signal intensity in OP-1group was higher than saline group at each time point after operation,which resulted in a significant Thompson score difference(P0.05)between 2 groups.Conclusion:OP-1 can delay the disc height loss and degrade MRI T2-weighted signal intensity after nucleus pulposus aspiration.
