Clinical application of four Y-V plasty for adult phimosis

目的 探讨四Y-V成形术治疗成人先天性包茎的临床效果.方法 自2008年4～10月,应用四Y-V成形术治疗成人先天性包茎患者63例,并观察术后包皮的上翻性、外观以及并发症情况.结果 术后5～14 d消肿（平均8 d）,7～18 d伤口愈合（平均11 d）.随访63例患者4～6个月,其中12例（19.1%）患者发生轻度瘢痕挛缩,均经局部外用激素后缓解;所有患者的包皮均能顺利上翻,而且外观满意.结论 采用四Y-V成形术矫治成人先天性包茎具有良好的疗效,在不切除包皮的前提下,患者术后可以获得接近正常的包皮功能和外观.

Objective To investigate the curative effect of four Y-V plasty for adult congenital phimosis. Methods Sixty-three patients with adult congenital phimosis were treated with four Y-V plasty from Apr. to Oct. 2008. The retract ability, cosmetic results and complications were observed postoperatively. Results The mean time of detumescence and wound healing after the operation were 8 and 11 days. After 4 months following up, all patients were satisfied with appearances and smooth retraction of prepuces except 12 cases（ 19.1% ） with slight cicatricial contracture which were cured by local steroid application. Conclusion Four Y-V plasty is an excellent therapy for adult phimosis without prepuce excised which provides near normal prepuce appearance and function.
