非肌层浸润性膀胱尿路上皮癌复发相关因素分析

Research on Influencing Factors of Recurrence of Non Muscle-Invasive Bladder Urothelial Cancer

【目的】探讨影响非肌层浸润性膀胱尿路上皮癌复发的相关因素及其处理对策。【方法】回顾性分析我院2003年1月到2008年12月161例非肌层浸润性膀胱尿路上皮癌患者临床随访资料,所有病例行经尿道膀胱肿瘤电切术,据术后复发率分析影响复发相关因素。【结果】161例中,45例术后复发,术后的总复发率为27.95%。肿瘤分级为G2、G3级患者较G1级患者复发率高;肿瘤分期为T1期患者较Ta期患者复发率高;肿瘤多发患者较肿瘤单发患者复发率高;复发肿瘤患者较初发肿瘤患者复发率高(P<0.05)。肿瘤大小、生长部位及术后用不同药物灌注与肿瘤复发无明显相关(P>0.05)。【结论】肿瘤分级、分期、肿瘤多发以及既往复发情况是影响非肌层浸润性膀胱尿路上皮癌复发的重要因素。

【Objective】To study the influencing factors of the non muscle-invasive bladder urothelial cancer and discuss the appropriate treatment.【Methods】Integrity parameter data were collected from 161 patients with non muscle-invasive bladder cancer in our hospital.They were followed up and the analysis of recurrence rate was performed.【Results】Forty-five of these 161 patients had tumor recurrence,the total recurrence rate was 27.95%.Patients whose tumor grade were G2 or G3 compared to those with G1 tumor,patients whose tumor stage was T1 compared to those with stage Ta tumor,patients with multiple tumors compared to those with solitary tumor,patients who had prior recurrence compared to those with primary tumor had greater recurrence rate（P0.05）.Tumor size,tumor location and intravesical instillation after operation with different chemotherapy drugs had no significant correlation with tumor recurrence.【Conclusions】Tumor grade,tumor stage,number of tumors and the prior recurrence condition are important influencing factors of the superficial bladder cancer.
